I, the undersigned, hereby grant Flushing Family Acres, its legal representatives, and assigns, the irrevocable right to use photographs of me and/or my family (including minors listed below) taken at Flushing Family Acres on ____________, for use in its promotional materials, including but not limited to:

• Website
• Social Media Platforms (such as Facebook, Instagram, Twitter, etc.)
• Brochures
• Advertisements
• Other forms of promotional media

I acknowledge that these photos may be used for marketing, promotional, or commercial purposes without further notification or compensation. I waive any rights to royalties or other compensation arising from the use of the photographs.

I acknowledge that the date of this agreement corresponds to the date on which I am submitting this form electronically.

By signing below, I confirm that I am of legal age or have parental/legal guardian authority over minors listed below, and I consent to the above release on their behalf.
